ENROLLMENT INFORMATION
The front office has the required registration forms that need to be completed in full. In addition, new students are required to provide:
* legal proof of age (e.g., birth certificate, passport)
* Certificate of Immunization - HRS Form #680
* Blue health forms completed, stamped and signed by physician, including the varicella portion on the back of the form.
* 2 proofs of residency (e.g., water bill, FPL bill, cable bill). If renting, please submit a cop
